J.; Schulman, A.; Grove, H.Oral cholecystography and ultrasonography of the gallbladder were performed in 190 patients with the aim of diagnosing gallstones. At surgery gallstones were found in 23 patients and a roundworm in 1 patient. The results obtained with the two methods are comparable. Since neither radiation nor contrast agents are employed in ultrasonography, it is suggested that this be regarded as the primary special examination in the diagnosis of gallstones.
- ItemObstruction-strangulation of post-traumatic diaphragmatic hernia : delayed diagnosis and fatal outcome : a report of 9 cases(HMPG, 1985) Schulman, A.; Fataar, S.; Alheit, B.The radiological and clinical features of 9 cases of obstructed post-traumatic diaphragmatic hernia are reviewed. In none of these patients was the diagnosis of a diaphragmatic hernia considered before radiography, all cases being clinically diagnosed as 'acute abdomen', most frequently pancreatitis or perforated peptic ulcer. Even after radiographs had shown opacity at the left base in all 9 cases, together with a distended proximal bowel in 6, the correct diagnosis was made in only 4. Through lack of correct radiological diagnosis, there was a delay of at least 1 day between admission and operation in 6 patients, and 3 of the 4 deaths occurred in this group. Obstruction-strangulation of diaphragmatic hernia should be considered as a possible cause of 'acute abdomen' in the presence of a left basal abnormality, especially in patients from areas with a high incidence of assault. confirmatory contrast studies should be done only if they can be done immediately and quickly. Pregnancy can cause hernias to become obstructed and strangulated, but this can be averted by obtaining a history of previous trauma early in the pregnancy.
- ItemPseudosolid appearance of simple and echinococcal cysts on ultrasonography : a report of 2 cases(HMPG, 1983) Schulman, A.; Van Jaarsveld, J.; Loxton, A. J.; Grove, W. H.The 2 cases reported here indicate that on ultrasound examination uninfected simple and echinoccocal cysts can sometimes show a pseudosolid internal consistency like that of a neoplasm, probably owing to floating crystals or particles. This has important implications as far as further diagnostic and therapeutic measures are concerned. The presence of a regular and well-defined rim remains an important clue to the possible cystic nature of such lesions.
- ItemSonographic diagnosis and follow-up in a patient with pancreatic roundworms : a case report(Health & Medical Publishing Group, 1989) Schulman, A.Ultrasonography showed roundworms (Ascaris lumbricoides) in the main pancreatic duct of a child with acute pancreatitis. This method was used to follow-up gradual reversion to normal during medical treatment.
- ItemSonographic diagnosis of biliary ascariasis(1982) Schulman, A.; Loxton, A. J.; Heydenrych, J. J.; Abdurahman, K. E.In a prospective 6 month study, sonographic diagnosis of biliary ascariasis was made in 12 patients: In five, the diagnosis was confirmed by other means, mainly intravenous cholangiography. In three, such confirmation was not sought, but all had proven intestinal infestation. One possible and three definite false-positive diagnoses were made. There were no established false-negative diagnoses. The echogenic, nonshadowing images of the worms were seen in the main bile duct and/or gallbladder as single strips (on one occasion with its digestive tract seen as an anechoic 'inner tube'), as multiple strips giving a spaghettilike appearance, as coils, or as more amorphous fragments. Follow-up sonograms were obtained in six patients and showed expulsion of the worms by medical treatment.
